The University Medical Center Utrecht - UMC Utrecht - is one of the leading academic medical centers in the Netherlands. It is a colorful organization that emerged in 2000 from a merger of the medical faculty of Utrecht, Utrecht University Hospital and the Wilhelmina Children's Hospital. UMC Utrecht is a complex task. It wants to be more than the sum of its parts and bring education, research and patient - three features that we now consider to be the pillars of academic medicine - into a single connection, the belief that this provides added value. The "UMC-thought 'was developed in the late twentieth century, partly because the parties believed in it, partly forced by circumstances. In a system marked by hyper-specialization, advancing technology and costs grew the need for a structure that would keep under control the expansion. The formation of the UMC Utrecht in a broader context of 375 years of post academic medicine, Annemieke Klijn shows the interrelationship between education, research and patient care is not an obvious constant. Until the mid-nineteenth century had care, education and research have little or nothing in common. Urban hospitals were institutions of poor relief, the university was only an educational institution, while research was a matter of passionate individuals and later took place in learned societies. Not until the last quarter of the nineteenth century became patient care, teaching and research step by step intertwined. With the creation of interfaces, however, the tensions increased, the various interests were not always parallel, the necessary discussions and even battle led. It is precisely these strains that make the history of academic medicine so lively and compelling. For where those discussions were going on and what efforts have been undertaken in the course of history to eliminate those tensions? Het Universitair Medisch Centrum Utrecht - UMC Utrecht - is een van de grootste universitaire medische centra van Nederland. Het is een kleurrijke organisatie, die in 2000 voortkwam uit een samengaan van de Utrechtse medische faculteit, het Academisch Ziekenhuis Utrecht en het Wilhelmina Kinderziekenhuis. Het UMC Utrecht staat voor een complexe taak. Het wil meer zijn dan de som der delen en brengt onderwijs, onderzoek en patiëntenzorg - drie functies die wij nu als de pijlers van de academische geneeskunde beschouwen - samen in één verband, vanuit de gedachte dat dit meerwaarde oplevert. De `UMC-gedachte' kwam tot ontwikkeling in de late twintigste eeuw: deels omdat de betrokken partijen erin geloofden, deels daartoe door de omstandigheden gedwongen. In een stelsel dat werd gekenmerkt door hyperspecialisatie, oprukkende technologie en kostenstijging groeide de behoefte aan een constructie die de expansie beheersbaar zou houden. Door de vorming van het UMC Utrecht in een bredere context van 375 jaar academische geneeskunde te plaatsen, laat Annemieke Klijn zien dat de verwevenheid tussen onderwijs, onderzoek en patiëntenzorg geen vanzelfsprekende constante is. Tot het midden van de negentiende eeuw hadden zorg, onderwijs en onderzoek weinig of niets met elkaar te maken. Stedelijke gasthuizen waren instellingen van armenzorg, de universiteit was uitsluitend een onderwijsinstelling, terwijl onderzoek een zaak was van bevlogen individuen en later plaatsvond in geleerde genootschappen. Pas vanaf het laatste kwart van de negentiende eeuw raakten patiëntenzorg, onderwijs en onderzoek stapje voor stapje met elkaar verstrengeld. Met het ontstaan van raakvlakken namen echter ook de spanningen toe: de diverse belangen liepen niet altijd parallel, wat tot de nodige discussies en zelfs strijd heeft geleid. Het zijn juist deze spanningen die de geschiedenis van de academische geneeskunde zo levendig en boeiend maken. Want, waar gingen die discussies over en welke pogingen zijn er in de loop van de geschiedenis ondernomen om die spanningen weg te nemen?
